Dental Enrollment - Voluntary Plan

**Check out the Important Dates for enrollment periods and policy dates.**

• Undergraduate or Graduate students who are eligible to enroll in the UHCSR medical insurance plan may choose to enroll in the voluntary dental plan.
  • Eligible dependents (legal spouse, civil union spouse, domestic partner, and dependent children up to age 26) may also be enrolled in the voluntary dental plan.

Graduate Assistants with waiver-generating appointments are automatically enrolled in the dental plan upon the completion of their appointment papers.  Such students may choose to enroll their dependents in the dental plan at their expense. 

Enrollment Instructions

• Student Coverage:

  • Go to the UHCSR Website.  

  • Select Explore Policy for the dental plan that aligns with your current enrollment.

  • Follow the on-screen instructions.

    • You may choose Continue as Guest; login is not required to make the purchase. 

  • Payment must be made upfront and is not assessed to your tuition and fees.

  • ***Note: When enrolling in the dental plan, you may also add eligible dependents at that time. Dependent coverage cannot be purchased separately - since dependents are not policyholders, the student must be enrolled as well.

• Dependent Coverage:

  • Undergraduate and Graduate Students: Use the Student Insurance Form above. 

  • Graduate Assistants: Use the Graduate Dependent Enrollment Form.
